Create a Google Form, add questions & collect emails
Google Forms lets you build surveys, order forms, event registrations and feedback forms, then collect the answers in one place. The form is saved in your Google Drive.
Create a form
Section titled “Create a form”Start with a blank form and give it a clear title.
- Go to forms.google.com and click Blank form. In Google Drive, you can also click New > Google Forms.
- If a Gemini panel offers to create the form for you, close it to start from scratch.
- Click Untitled form and type a title, such as “Customer feedback”.
- Click Form description and add a short explanation for the people filling it in.
Add questions
Section titled “Add questions”Each question has a type that decides how people answer.
- Click Untitled Question and type your question.
- Click the question type list on the right and choose a type.
- Add answer options if the type needs them.
- Turn on Required at the bottom if the question must be answered.
- Click Add question (the plus icon on the right) for the next question.
These are the most useful question types for businesses:
- Short answer: a name, phone number or order number.
- Paragraph: longer comments or complaints.
- Multiple choice: pick one answer, such as a branch or a size.
- Checkboxes: pick several answers, such as the products someone is interested in.
- Dropdown: pick one from a long list, such as a state.
- File upload: attach a receipt or a document. Responders must sign in to a Google Account.
- Linear scale or Rating: a score, such as 1 to 5 stars.
- Date and Time: an appointment or delivery slot.
Collect email addresses
Section titled “Collect email addresses”Recording email addresses lets you reply to people and stops anonymous answers.
- Click the Settings tab at the top.
- Click Responses to open that section.
- Under Collect email addresses, choose an option:
- Verified: people must sign in to a Google Account, and their account email is recorded.
- Responder input: people type their email address themselves. Best for customers who may not use Google.
- Do not collect: the form stays anonymous.
- Optionally, set Send responders a copy of their response to When requested or Always.
In the same section, Limit to 1 response stops people answering twice. It requires responders to sign in to a Google Account.
Publish & send the form
Section titled “Publish & send the form”People can only answer after you publish the form.
- Click Publish at the top right.
- Under Responders, check who can answer. It may be set to people in your organisation only. Click Manage and choose Anyone with the link if customers or suppliers need to answer.
- Click Publish.
- Click Copy responder link at the top and paste the link into an email, WhatsApp message or your website.
Before you publish, click Preview (the eye icon) to try the form yourself. To see the answers, go to view Google Forms responses.

Common issues
Customers outside my company cannot open the form. Why?
The form is probably limited to your organisation. Click Published, then Manage under Responders, and change the access to Anyone with the link. If that option is missing, your admin has blocked sharing outside the company.
Why can't I add a File upload question?
File upload does not work when the form is saved in a shared drive, or when your admin has certain data protection rules turned on. Responders must also sign in to a Google Account to upload, so it is not ideal for customers without Google accounts.
Can I edit the form after people have started answering?
Yes. Changes apply to new responses. Avoid deleting or renaming questions that already have answers, because it makes the results harder to read.
